Call for Expression of Interest & Privacy Notice

Thank you for your interest in the Medigas Client and Family Advisory Committee (CFAC). This Privacy Notice describes your privacy rights and how we protect your privacy when:

1. You submit your interest to participate in the CFAC, and
2. You are chosen and you agree to participate in the CFAC.

Personal information we collect from you:

• If you submit your interest to become a CFAC participant, we will collect your name, address, email address, and telephone number so that we may contact you, if you are selected to participate in the CFAC.   
• If you are chosen, and you agree to participate in the CFAC via virtual meeting(s), you will be asked to describe your experiences and provide your opinions. Medigas staff managing the CFAC will compile this information in written notes, and sessions may be recorded via video and/or audio recordings. Although your experiences and opinions will be recorded without attributing your name and contact information to the information you share, it may be possible to identify you.

How we use your personal information:

The opinions and experiences of all participants compiled by Medigas staff managing the CFAC will be used by other authorized Medigas staff (in a format that does not directly identify you) for the following purposes:

• program and service design, planning and coordination;
• quality improvement initiatives;
• evaluation of processes and services, such as those related to client safety, infection prevention and control, client orientation, and training.

How we protect your personal information related to the CFAC:

Your personal information is protected using reasonable safeguards, such as those described in the Medigas Privacy Statement which can be found here.  

All personal information related to your participation in the CFAC will be used and stored within Canada. If you are a resident of Quebec, you should be aware that your information will be transferred to authorized Medigas staff outside of Quebec, but within Canada for the purposes described in this notice.  

Your privacy rights:

You have rights related to the personal information you choose to provide, including a right of access, rectification, and erasure, and the right to withdraw consent to the communication or use of the information we collect from you.  For more information on your privacy rights, including how to contact our Chief Privacy Officer, please refer to the Medigas Privacy Statement.

ACKNOWLEDGEMENT

The following information is provided to allow you to make an informed decision as to whether you wish to participate in the CFAC. Please read and acknowledge the following before submitting your request to participate in the Medigas CFAC:

I acknowledge that if I am selected, and I choose to participate in the CFAC:

• My participation is voluntary. The information I choose to share in the CFAC virtual meetings is at my complete discretion;
• I may withhold or withdraw my consent to participate in the CFAC at any time;
• The CFAC meetings will include up to twelve other participants (current or previous Medigas customers, family members or caregivers), and a maximum of three Medigas staff members;
• By participating in the CFAC and attending CFAC meetings, my identity may be known to the other group participants;
•  To respect the privacy of other participants, I agree that:

• By participating in a virtual CFAC meeting, I accept that there are additional risks beyond the control of Medigas. For example:

• I further acknowledge that Medigas is not responsible for any potential use, misuse, or disclosure by participants of any personal information I voluntarily share.
